Your Procedure Says One Thing. The Job Is Done Another Way.

The gap between work as imagined and work as performed is operational evidence. Treating every adaptation as misconduct wastes it.

Procedures are models

A procedure is a model of how work should be performed under anticipated conditions. It selects a sequence, identifies hazards and defines boundaries. Real work contains variation: equipment differs, access changes, materials arrive late, interfaces overlap and weather shifts. Workers respond to that variation because the job still has to be completed. The existence of adaptation does not prove recklessness. It may reveal expertise, a procedure defect, poor planning or pressure that the written system has failed to address.

Rules can help and hinder

Hale and Borys reviewed the complicated relationship between rules and safe work. Rules can preserve hard-won knowledge and create a common standard, but too many detailed rules can become unusable or internally inconsistent. A rigid rule may also remove discretion that is necessary in variable conditions. The answer is not to abandon procedures. It is to decide where prescription is essential, where principles and boundaries are more useful, and how the people doing the work can report mismatches without being automatically blamed.

Why deviation happens

Some deviations are deliberate shortcuts and must be addressed. Others occur because the specified tool is unavailable, the sequence cannot be followed with installed equipment, or simultaneous operations create a condition the author never considered. Supervisors can also normalize a different method while the document remains unchanged. Production incentives may quietly reward speed over compliance. Lumping all of these causes under ‘failure to follow procedure’ prevents the organization from distinguishing willful risk-taking from a weak work system.

Write with workers, not for them

Procedure development should include experienced users, maintainers, contractors and supervisors. Walk through the task where it occurs. Test whether steps are observable, controls are available and decision points are clear. Ask what changes between routine and non-routine conditions. A usable procedure tells people what must never vary, where they may exercise judgment and when work must stop for a new assessment. Participation improves technical quality and makes hidden workarounds discussable before they become normalized.

Create a feedback loop

A procedure should have an operational owner and a simple route for users to propose correction. Supervisors should capture recurring mismatches during pre-job briefs and post-job reviews. Changes with serious-risk implications require disciplined review, but minor usability corrections should not wait years for a major revision cycle. Measure repeated deviations by task and reason, not merely by worker. A cluster around one step is evidence about the system. It is not proof that many workers independently chose to be careless.

What leaders should do differently

Choose a sample of high-risk tasks and compare the written method with an observed job. Ask workers which steps add value, which are impossible, and which controls they protect even when production is difficult. Correct resources and planning before demanding perfect compliance. Define a fair response to intentional violations while protecting good-faith reporting of procedure problems. The objective is alignment: the safest practical way of doing the job should also be the documented, resourced and supervised way.

Observe where the procedure changes

Select a procedure that workers often adapt. Observe the task and ask experienced workers to explain each difference between the written method and the work. Check whether the gap comes from changing conditions, missing equipment, awkward sequencing or an outdated instruction. Correct urgent risk first, then revise the procedure with the people who use it.

Questions about the gap

Which steps do people change, skip or add? What problem are they trying to solve? Does the approved method still fit the equipment and work environment? Which deviation increases risk, and which one reveals a better method? Who owns the feedback and revision process?

Give workers a credible route to report it

A useful procedure reflects the job closely enough to guide decisions. Workers need a credible route to report when it no longer does.
