Life-Saving Rules Need Enabling Conditions, Not Slogans

Clear rules for fatal risks are useful only when the organization provides the equipment, planning, supervision and authority needed to comply.

Keep the purpose clear

IOGP’s Life-Saving Rules focus attention on activities repeatedly associated with fatalities. Their value lies in a concise common language for critical actions such as verifying isolation, protecting against falls and staying out of the line of fire. They are not a complete safety management system and do not cover every occupational health or process safety risk. Organizations should present them as a focused layer within a broader risk-control framework.

Translate without distorting

A global rule needs local meaning. Explain how each rule applies to the site’s equipment, permits, roles and legal requirements. Use real task examples and competent translation rather than word-for-word text that workers may not understand. Avoid adding so many local rules that the simplicity disappears. Where a local standard is more stringent, make the relationship explicit. Contractors need the same operational interpretation, not a separate poster with different terms.

Provide the conditions to comply

A worker cannot verify isolation without accessible points, correct drawings, locks, instruments and enough time. A driver cannot follow journey controls under an impossible schedule. A person cannot stay out of a line of fire where layout forces access through the danger zone. Before enforcing a rule, leaders should test whether design, planning, staffing, equipment and supervision make compliance practical. Repeated breach often signals a weak system condition that management owns.

Connect rules to critical controls

Rules describe essential human actions, while critical-control management defines the barriers and performance requirements around the risk. Link each applicable rule to the organization’s material risks, permits and verification process. For work at height, the worker action sits alongside anchor integrity, equipment inspection, rescue readiness and competent planning. This connection prevents the rule from becoming a behavioral slogan detached from engineering and organizational controls.

Respond fairly to breaches

A breach with fatal potential demands immediate control and a careful review. Establish what happened, whether the rule was understood and workable, what supervision and resources were present, and how similar cases were treated. Intentional reckless conduct differs from error, ambiguity or an adaptation required to get the job done. Automatic dismissal policies may appear decisive but can discourage reporting and hide the conditions that will expose the next crew.

Verify use in the field

Do not measure success by posters distributed or training completion alone. Observe high-risk work and ask workers to explain the relevant rule, hazard and control in their own words. Review stop-work cases, repeated deviations and control failures by rule. Test whether supervisors protect people who pause work. Assurance should show whether the required action and supporting barrier are present at the moment of exposure.

Leadership sets the boundary

Senior leaders influence whether rules survive schedule and cost pressure. They should ask where compliance is difficult, fund removal of recurring constraints and refuse to reward output achieved through degraded controls. When work stops, leaders should focus first on making it safe and learning why. A Life-Saving Rule becomes credible when the organization accepts the operational consequences of applying it.

Follow one rule into the work

Take one Life-Saving Rule and follow it into a real task. Ask workers to explain the rule in the context of that job, then check whether the equipment, planning and supervision allow them to comply. Record any conflict between the rule and the work. Fix the condition before treating the problem as a communication failure.

Questions behind the rule

Which fatal risk does this rule address? What critical control sits behind it? Can every affected worker comply under today's conditions? What happens after someone stops the job? How does the organization distinguish an honest mistake from a conscious breach?

Make compliance possible

Rules set clear boundaries, but leaders must provide the conditions, equipment and authority needed to follow them.
